PMH Self-Assessment:
Compare your results

As part of the PMH website update, the self-assessment tool has been updated with an important new feature. Now, when taking the self-assessment you can compare your current results with a previous session. By entering your email when taking the test, the tool will send you a reminder to self-assess again after some time. When you come back again, your most recent results will be compared to your past results to show you the progress to date on adopting the PMH model. Remember, you can take the self-assessment any number of times, and with any frequency. The reminder and comparison functions of the new tool are designed to give you better, more progressive information about your PMH journey.

CFPC’s Best Advice Guide: Chronic Care Management in a Patient’s Medical Home provides actionable advice to family physicians about chronic care management in family practice settings. While this guide applies to all types of family practices, the strategies described involve many important components of the Patient’s Medical Home model. With proper intervention and ongoing care family doctors can help prevent and control the impacts of chronic diseases by implementing practical chronic care management strategies in their practices.

On May 27th, the Ontario Chapter of the College of Family Physicians of Canada, with the support of The College of Family Physicians of Canada, sponsored a special Symposium on the Patient’s Medical Home. The event was very successful, allowing family physicians to engage in important discussions to further develop directions to advance the pillars of the Patient's Medical Home (PMH) in Ontario's evolving health system.
