LEAD STORY - New models of care for managing Chronic and Complex Health conditions

In this month’s eNews we look at a recently released report titled Better Outcomes for people with Chronic and Complex Health Conditions. Amongst the recommendations from the report’s author Primary Health Care Advisory Group (PHCAG), is a proposed model of care for Australians with chronic and complex conditions. In Sydney’s north, work is underway in this area to improve patient health outcomes for at risk groups.

  • A skill training session in recognising, responding and referring cases of domestic and family violence is being offered this month and a list of resources is also available.
  • GPs can refer their patients with asthma to The Asthma Foundation’s telephone asthma education, support and follow up.
  • A new four-strain flu vaccine, protecting against the nasty Brisbane virus, is being made available for at-risk Australians for free from this month.
  • The National Disability Insurance Scheme (NDIS) is hosting information sessions to help people with a disability, as well as their families and carers, get ready for the NDIS that will commence on 1 July 2016.
  • In Aged Care services news, the NSLHD has created a GP resource for referring people to the program.
  • In type 2 diabetes news General Practices are invited to take part in a study that tests whether an online self-help program can help people with type 2 diabetes feel happier and be more active.
  • In Palliative care news CareSearch is providing information to GPs to help with sensitive discussions around palliative care.
  • Diabetes NSW has provided its calendar of education events for people living with diabetes.
