Welcome to WAPHA Connect, an update on all things happening in primary health care across Western Australia.

In this issue:

New chronic disease service for Midwest, Gascoyne and Murchison regions
New integrated chronic disease services in the Midwest, Gascoyne and Murchison regions will improve care for individuals at risk of chronic disease. The new services will see teams of health professionals travel to remote areas to coordinate care and support individuals to manage their chronic conditions, with GPs remaining at the centre of their patient’s care. Read more

WAPHA welcomes addition of metropolitan and regional AOD contracts

In March, the Department of Health announced it would transfer a number of existing agreements under its drug and alcohol treatment grant programs to Primary Health Networks. WAPHA recently met with relevant organisations to commence discussions on continuity of the programs. Read more

Review of aged care strategies for people from diverse backgrounds
The Federal Government is reviewing the national strategies that support the aged care needs of people from Culturally and Linguistically Diverse (CALD) and Lesbian, Gay, Bisexual, Transgender, and Intersex (LGBTI) communities and people across both sectors are being encouraged to put their views. Digital Health forum in Northam
WA Primary Health Alliance (WAPHA) and the Australian Digital Health Agency recently hosted a Digital Health Forum for health professionals in the Wheatbelt.  Read more

Expressions of Interest
WAPHA is currently providing opportunities for interested parties to participate in an Expression of Interest (EOI). There are currently three EOIs open:

  • Country WA PHN Innovations & Collaboration Small Grants
  • Country WA AOD & Mental Health Capacity Building
  • Integrated Chronic Disease Care - Pilbara
  • Integrated Chronic Disease Care - Southwest
  • Integrated Chronic Disease Care - Great Southern

Healthy in the Bush a success for Cranbrook
WA Primary Health Alliance (WAPHA) and Cancer Council WA recently hosted Healthy in the Bush, a community event held in the small community of Cranbrook and surrounding towns where locals had the opportunity to meet health providers and learn about services available in their areas. Read more
