As part of an ongoing effort to improve the health of residents in the Charleston area, please provide your feedback via this survey & share with your neighborhood. It should only take about 5-10 minutes to complete.

Community health needs assessments are designed to help improve the overall health and wellness of communities by identifying community needs, use of local health resources and action needed to address health care delivery in a defined area. The Eat Smart Move More Tri-County Coalition has been working together for several years to improve the health of residents in the Charleston area. They have worked on initiatives as it relates to school-based projects that addressed nutrition and physical activity as well as health of residents in faith-based communities and worksite wellness policies.
